This is a practical guide on how to legally get a Gender Dysphoria (GD Certificate) or Gender Identity Disorder Certificate (GID Certificate), although the latter term is quite archaic since the World Health Organization replaced the word "disorder" with "Gender Incongruence" in the International Classification of Diseases, 11th Edition (ICD-11)!


Why do I need to get a GD Certificate & what are its benefits?

A GD Certificate is an extremely important document that you will need throughout your transition, both legally and medically.

On the medical side, most registered medical practitioners will refuse to provide you with gender-affirming care if you fail to produce a GD certificate. From a legal perspective, you will need a GD certificate to obtain two of the most important documents for your legal transition, i.e., your Central Gazette notification and your Transgender Identity Card and Certificate (issued under Section 7).

However, do keep in mind that this applies ONLY to those who are interested in applying for a binary gender marker such as M or F. If you would prefer the TG or T marker, you aren't typically expected to have a GD certificate with you to legally transition. 


Who can legally issue a GD Certificate?

Legally speaking, either a clinical psychologist with a valid Rehabilitation Council of India (RCI) and who has experience of working with transgender folk or a psychiatrist can issue a GD certificate. The difference between the two is that a clinical psychologist is not a medical doctor and typically deals more with the therapy aspect of things, whereas a psychiatrist is a medical doctor who can legally prescribe medications and usually doesn't focus as much on the therapy aspect (though they often recommend it!). 

Practically speaking, a good chunk of registered medical practitioners will refuse you gender-affirming care despite having a valid GD certificate from a registered clinical psychologist, wrongly demanding that it be issued by a psychiatrist instead. Keep in mind that if you plan to undergo any gender-affirming surgeries, you may be required to obtain multiple GD certificates from a combination of clinical psychologists and psychiatrists. However, for Hormone Replacement Therapy (HRT), either one should suffice.

From personal experience, I have never had an issue accessing gender-affirming care with a GD certificate issued by a clinical psychologist, but I do know a few trans folks who have run into trouble with this.

In terms of legally transitioning, it does not make much of a difference whether the GD certificate is issued by a psychologist or a psychiatrist. I personally did not face any issues during my legal transition, and I am not aware of anybody else who has faced issues while legally transitioning using a GD certificate issued by a clinical psychologist.


Expected timeline and cost to procure a GD certificate

The amount of time it takes to obtain a GD certificate depends on the mental health care professional in question. Many queer-informed professionals may issue GD certificates more readily (often in a single session), whereas other mental health practitioners may require you to have multiple sessions with them before issuing one.

I was issued the GD certificate by a clinical psychologist in just one session. You can expect it to cost you ₹2,500, which includes both the session and the issuance of the document (it's an online certificate, which is completely valid).

Please be mindful that your costs and timeline may differ from mine and others, but just hang in there! Because trust me, it WILL happen.

What can I expect in the session?

In the session, you can typically expect questions regarding how you identify, your childhood, how long you've experienced dysphoria, how you feel when your dysphoria spikes, and general questions concerning your gender identity.

If they are a competent professional, they should NOT make you feel unsafe or uncomfortable. If you feel uncomfortable, it is their job to notice. If you communicate that you are uncomfortable and they still keep pushing, that is a RED FLAG. They may also ask about your relationship with your family, which may be mentioned on your GD certificate as well.

What are the documents needed to get a GD certificate?

All you require to obtain a GD certificate is your Aadhaar card. Often, they will mention your Aadhaar number on the GD certificate so that it is officially linked to you!

The GD Certificate typically includes your deadname, your chosen name, and your pronouns. It provides a brief explanation of your gender dysphoria and, towards the end, specifically gives the green light regarding your competency to consent to gender-affirming care, such as hormone replacement therapy.
